Good news! Danny Lau put me in touch with Jason Patton and this item is moving forward. From Jason:
"Your CityWorks request was routed to the Transportation Services Division and it is in their queue. I am getting involved to help resolve this issue because I was responsible for the striping plans that added the bike lanes to Shattuck Ave in conjunction with the resurfacing of the roadway. In our design process, we did not include the “keep clear” marking. That marking was initially installed around 2005 and, by the time the bike lane project was in design, the “keep clear” marking was no longer visible:

I will be supporting the Transportation Services Division in completing the work order to have the marking installed. The work order is then submitted to the Bureau of Infrastructure and Operations – the organization that has a striping crew. Note that their queue is also quite long. So the short answer is I will help get the work order submitted, but I am not able to give you a timeframe for final resolution of the issue."
