Description
Just shredded my sidewall of my car - huge pothole in the middle of the road of Escalante on south side of the road - now have to get tire replaced....this road is a disgrace and this is the second time I've had to replace tire because of potholes in Tucson - who reimburses me for this?

A City of Tucson claim form with instruction sheet is available by fax, email or mail.
Please call Risk Management at 520-791-4728 to request your form and get more details. All Claim Forms must be hand-delivered to City of Tucson, City Clerk’s Office, 255 W. Alameda, 9th Floor, Tucson or mailed to City of Tucson, City Clerk’s Office, PO Box 27210, Tucson, Arizona 85726-7210. No claim forms can be emailed or faxed.

To make that happen we are closing out all issues that are over 6 months old. If the issue we are closing out was never fixed, or if it has recurred since you reported it and it was fixed, please report it and we will have it in the new system.
