Sewer cover rattling out in front of #80&83 winter street

Cover was loose and dislodged - now cover missing

Utility cover is a hazard for walking and disability devices.

Across the street from 327 Lincoln Av is a new storm drain installed last summer along with new sidewalks. The storm drain is always full and seems to be flooding the street and causing water to run down to Nason Rd to another storm drain.

The cap for the water shut off in the street, directly in front of the new hydrant is missing causing a large hole in the road.

Not sure if cover issue or sidewalk, either way the cover is raised and sidewalk a hazard. Across from Lewis St.

Kowloon lot, front (Broadway) side, broken water access cap (frame). Protruding from ground (pedestrian tripping hazard). Can height be fixed and / or an asphalt patch be added to make it level the lot around it? Again (esp and night) it’s a pedestrian safety hazard. Thank you

Water utility cover or base is loose and rattling when vehicles pass over it. Location is in eastbound lane of Ballard Street at intersection of Houston Avenue and Ballard. For identification purposes this cover has been painted red.
