No mask mandate, no sign on door, patrons unmasked, my questions to management ignored..... they are also open during certain hours with no staff on duty to enforce mandate (key entry my patrons) .....Foundation Fitness

the business is not following and licensing protocols and or covid. i pulled my kid out of this center due to that. the children who are 5 years old do not wear mask as supposed to while entering the center they have 2 , 3 ,4, 5 in the same classroom the ( 3 year old room) they hired an staff member who had covid 19. and brought it in the center with them knowing. the center needs to be shut down. the director never wears a mask when she is there. each time i drop my kid off she quickly puts it up.

I want to report this business at risk of exposure of COVID-19. I had an appointment there, when I got there, it's full of people and crowded. There's no social distance guide signs and they didn't follow the restriction to reduce the maximum occupancy. All of their rooms were occupied, I had to cancel my appointment after I saw that.

Improper use of face mask inside building. Nose not covered

The indoors facility where the Ice skates are rented was packed with people on 1/9/22 with no masks. There were approximately fifty people indoors (renting and changing their shoes), with 97% of them with No Masks. It's incredibly disappointing to see this behavior from people but even more by parks and recs. to have no control. As health care worker is heartbreaking to see the management of this facility in a middle of a pandemic. I hope this will change immediately.

A provider at the facility is KNOWN Positive COVID 19 and still working with patient. The facility also does not follow the 6ft apart guidelines.

While visiting the northern recycling facility at: 100 Dover Road North East
Glen Burnie, MD 21060 today January 8, I noticed none of the employees were wearing mask. I ask the guy at the gate if mask have to be worn he said yes. But he was not wearing one , his nose is running and he is coughing he was noticeably sick and is right in my window looking to look at license and registration. He also informed me that they no longer use the stickers on the windshield to be allowed to come in. This is the worst time to stop the sticker , now everyone has to interact with this person instead of keeping the window up while we are in the middle of a pandemic. Then after getting inside the recycling area I saw three more employees with out a mask. The guy I had to talk to about what I’m bringing in had no mask.

If the MVA is going to use one cleaning wipe after each customer to clean the touch-screen monitor, credit-card machine and eye test at a given station, please clean the eye test first, not last. (This was observed at the Glen Burnie office on 08/15/20.)

Went for a service nobody following the executive order. Meet with older sales manager named Dennis , his exact words I am here 39 years We are not required to follow the orders.

Customers not wearing face masks. No social distancing.

Sales Manager traveled overseas and come back without 14 day period. Sales person and service advisor took 5 days off and they forced them to take 14 days quarantine period. Our lives is in danger under the executive management.
They are not following any orders of the state and the country.
