    On December 31, 2021, I went to the AFC Urgent Care facility to receive a drive-thru PCR covid test. I found this facility on the Anne Arundel County Website as a facility providing no-cost testing. When I got the test, I was told I would receive my results in 24-48 hours. A few days later I received an email from AFC Urgent Care stating that they would need a picture of my driver's license and insurance card to proceed with my results. I have been tested other times in the county and never been asked for my insurance card and can't remember if anyone asked to even see my license, but was never asked for a picture. This is potentially vulnerable personal information and I am extremely uncomfortable with this demand.

    It has been 11 days since I had my covid test and still don't have any results. I believe the facility is refusing to give me my results without the license and insurance cards. I can't believe this is legal and am asking for help getting my results. Thank you.

    At approximately 6:10pm on 12/17/2020, an employee of Merritt Athletic Clubs sent another member of the gym into an area to workout that was within 2 feet of myself and another gym member. When we asked the employee what he was doing, he told us we could leave if we had a problem.

    I currently work in the shop part of this business, on Tuesday 3/16 we were told an employee tested positive for Covid. To let management know if we feel we need to get tested or if we feel we were in close contact with this person, this person performs different jobs all over the shop. I though oh wow this is the first time I felted they handle it great. As I started to get more details I realized that wasn’t the case. What I heard/ did see the employee was at work on Monday for a good 4 hours mind you all over the shop, at that point the employee reports to the supervisor that his mother tested positive for Covid which he lived with, he was tested over the weekend was positive for Covid. The supervisor send him home immediately. Fast forward to yesterday 3/17 the employee is back at work with only being out for a day and half. I was told he had a note and could return because he had no symptoms that there was nothing they could do. I’m beyond appalled by this company for letting this employee back into the work place I don’t care if he has a note or not first he was exposed which should be a 14 day quarantine. Then he tested positive which should be a 10 day quarantine. Even if he is asymptomatic. We have been open the whole time in the past year and I have disagreed with a lot of things and how the company handled Covid procedures between cleaning ( we when days without working toilets but they got Porter potty‘s there within hours so they didn’t have to shut down they were so gross) and not screening properly when employee come to work. ( all that is done is our temperature and we are asked if you are ok) But I can’t over look this reason incident this shop needs to be looked into I don’t feel safe coming to work I’m worried that I could bring this home to my family all because they didn’t follow the proper protocols or CDC guidance. Thank you for everything I hope this is thoroughly looked into.

    Not wearing facemasks. Individuals in club not covering nose and mouth with facemask. Including group exercise instructor on multiple occasions, not covering nose and mouth with mask. Mask is on her chin when she is teaching cycling classes.

    There is a hugs over masks freedom festival at the lake where strangers plan to go out in public and hug each other without masks on 10/24 between 4pm and 6:30pm. This is why our COVID numbers are increasing! Please stop this!

    Panthers football practices - very few players or families or coaches are adhering to county guidelines on mask mandates (all should be wearing to and from the field), coaches should have no more than 10 players assigned to them, and parents should maintain social distancing. This organization thumbs their collective nose at the rules in place to protect our county's citizens. DO SOMETHING!!!!

    Despite the Mask mandate in Md Pic N Paint has made it optional for guests to wear masks which is unacceptable since its putting others at risk but especially since they’re mandates in place in Maryland to stop this from happening to protect everyone. I left right away when I saw this because physical distance doesn’t do much of anything it isn’t paired with the other precautions.

    1403 Madison Park Drive: STE # 100 = Dr. Shahab A Malik & Associates (Pediatrician Office), LLC had (2) employees test (+) for COVID-19 in the last 1 week. The office is not being properly cleaned. Face coverings not enforced, and removed once in office, and there are often > 10 - 15 people waiting in close proximity / not following social distancing guidelines in the waiting room. The office is not being properly cleaned. no professional cleaning is being done. The office was not shut down for cleaning when the employer was notified that his staff members were testing positive for COVID-19. Aerosalizing procedures are being done on a regular basis, risking the further spread of COVID19. Patients with COVID-19 symptoms are often seen in the office by the staff and not asked to partake in tele-health visits as reccomended by the CDC. Patients are coughing in the waiting room around others. Patients are not being consistently screened appropriately for symptoms. The office continues to see > 300 patients + parents /week and often -times > 100 patients in 1 day, not allowing for proper social distancing & patient occupancy control

    If the MVA is going to use one cleaning wipe after each customer to clean the touch-screen monitor, credit-card machine and eye test at a given station, please clean the eye test first, not last. (This was observed at the Glen Burnie office on 08/15/20.)
