Unsafe sidewalk

Impossible to make it safely up the hill via the sidewalk utilizing stroller or wheelchair

Good Afternoon. The sidewalk in front of my home at 87 Lent Road, Hamden, CT 06517 has several crack and divots in it. It is a trip and fall hazard. I do my best to trim it and keep the grass from growing out of the cracks, but the cracks are getting larger and larger each year. My mother is 81, and I am always nervous maneuvering her to car for her medical appointments as we have to pass by these cracks in the sidewalk. Anything you can do to repair these sections of sideway would be greatly appreciated. I have two (2) additional photos I can send over as well. Thank you for your time and consideration.

4/7/2021-I have reported a problem with the sidewalk in front of my home since Feb 2011. The sidewalk is raised due to tree roots that the town has since taken down (Oct 2013). The sidewalk has cracked more and when it rains there is a huge puddle and now it's completely crumbling to small rocks. Many older and handicapped people walk on this side of the street and I have seen many people trip. The town has marked out the sidewalk and driveway skirt numerous times, needs repaired asap.
Here is the last signoff from the Engineering Inspector 12/8/2017: "The Town currently repairs sidewalks based on the roads with the oldest complaints. Once we get to a street, we attempt to repair all sidewalk problems on the street. We are currently working on streets with complaints from 2007 and about to begin 2008 streets. Based on current funding and given your street is a 2007 street, we anticipate it to take within a year to get to the street. The town is currently winding down sidewalk work for the year due to impending cold temperatures and snow". I do not understand why it would take over 10 years for the Town to repair this issue. I would appreciate a timely response.

There is a sidewalk from the beginning of Denslow and stops in front of 148 on the eastern side of the street. An 81 year old resident walks it in the am and so do the neighbors in there 80's.
The sidewalks need repairs. They are lifted from the tree roots and are a tripping hazard.

Here is a photo of the grassy area needed to be fix along with the cracked and crumbled sidewalk in front of the house on Ford street the whole sidewalk in front of 70 Ford as well as the house next door needs to be replaced or repaired as soon as possible as it is dangerous along with tree stump grass and roots left after the private contractor removed the town tree off of both houses.

Other side of the road sidewalk is affected and unsafe to walk on.
