 Time of Incident: Approximately 3:20 PM
 Location of Incident: Sidewalk in front of 92 Duncan Avenue, near the intersection of Kennedy Boulevard, Jersey City, NJ
    Description of Incident:
On October 31, 2025, after shopping for my mother at C-Town Fresh Market located at 759 West Side Avenue, Jersey City.
    While walking along Duncan Avenue, near my mother’s residence, I tripped and fell on the sidewalk in front of 92 Duncan Avenue, which is uneven, damaged, and lifted, creating a trip hazard. The fall occurred next to the parking lot area near where my mother lives.
    As a result of this fall, I twisted and severely injured my left foot and ankle. I was transported to the medical center, where I am now preparing to undergo surgery on my left leg due to the injuries sustained from the fall.
    The sidewalk at 92 Duncan Avenue is in poor and unsafe condition and presents a serious hazard to pedestrians. I am requesting that this hazardous sidewalk be repaired immediately to prevent future injuries.

    New development was built adjacent to 116 Monitor Street. They installed heated pavements, but did not slope the pavements or put drainage, so water immediately refreezes onto 116 Monitor property front walkway, creating ice and hazard. This is essentially the same as your neighbor shoveling snow off his propery and putting it on your property. This is the third time this construction company has caused damage to my property.
