Significant dip surrounding a raised manhole

Q. Is the hole an even square shape (indicating a trench from utility work)?
A. Not a hole but there wasn't an option that better matched.
Q. How deep is the pothole?
A. Raised a few inches compared to surrounding pavement that dips a few inches below the rest of the road surface
Q. How big around is the pothole?
A. About 1-2ft around the manhole is depressed.
