This is not resolved. This is runoff from the neighbors house(556 westminster hill rd) who put in a rock driveway(which i don't believe a permit was pulled for). My driveway is a small stream on most days due to this runoff. This issue was brought to the city's attention over the winter when I ended up with over 2 feet of ice in the driveway, and I was told he was ordered to fix the issue, but it has not been fixed and has caused multiple potholes between his house and at least mine.

Your issue has been assigned to a DPW employee for inspection. Once the scope and extent of the issue is determined, it will be scheduled for maintenance or repair as needed.
