I am looking to get certified date of birth certificate of my daughter born in Framingham ( Metro west medical center). Can I apply online or is there any link to place my request?

Hello,
There are 2 ways that you can request a birth certificate. You can either come into the office and receive it right away or mail in a request. If you wish to mail in a request please include the name and date of birth, $15 check or money order and a self addressed stamped envelope. If the parents were not married at the time of this persons birth then please also include a photo copy of your ID.
Mail the request to Framingham Town Clerk, 150 Concord St., Framingham, MA 01702
