I would like to know the procedure how to apply for copy of birth certificate of my daughter born in Framingham metro west hospital. Currently I am residing in Mooresville, NC 28117.

Hello,
Since you are not in the area you can mail in a request to our office including the name and date of birth, $15 check or money order and a self addressed stamped envelope. The turn around time is 1-2 days. If the parents were not married at the time of the birth then please also mail in a photo copy of your ID along with the written request.
