21st Street ceases being a state maintained roadway at Wall Avenue (SR-204). You will need to contact Ogden City via their street damage hotline at (801) 629-8990.

They may have told you this, but this portion of 21st Street is not maintained by UDOT. So Ogden City is the organization that needs to repair it.
