What is done then if this is a duplicate issue? (Must be a problem if it keeps reoccurring)

If an issue has already been reported and there is still an active case we mark it as a duplicate. That ties the new report to the original so every link goes back to the page with the case information. That way we don't have multiple pages that need updating. If additional information is provided it is added to the case information to assist the investigator. We can view all of the reports and unduplicate them if necessary if a new case is started from the information provided.
