Hello - thanks for the update. The cause of the defect at this location is a failing privately-owned drainage pipe that runs under the sidewalk and was improperly broken into the inlet structure. This pipe does not appear on our record documents for this neighborhood and is not associated with a maintenance easement.
The residents abutting this location have been notified of their responsibility to repair their pipe and the resulting damage it has caused to the sidewalk.

As previously stated, the sidewalk defect is due to a failed storm drain line from a back yard drain tied into the roadside inlet box. This line is outside of County right of way and not contained within a maintenance easement.
As such, we have no maintenance responsibility here under County Code 18.402(a) which limits our responsibility to those sidewalk panels damaged by a tree in County right of way or a County water/sewer project.
The property owners responsible for this repair to both the sidewalk and the drain line have been notified. I will forward this request back to our office to follow up with these residents.
