During City business hours, this should be reported to the Traffic Dept. by phone at 617-349-4700. If after hours, please call the Police Non-Emergency Line at 617-349-3300.

Thanks for the response and information to call Traffic/Police department. I thought the purpose of this form was to provide a means to contact the city in addition to calling? If so, isn't the onus now on Traffic/Police to follow-up with the information provided (ie visitor pass info.) towards a resolution which would be captured and tracked here before the issue is closed? Thanks for your help.
