Description

Manhole cover is loose and does not cover entire hole. I fell through when stepping on the cover. It is not the correct size for the hole.

Q. Please describe in detail the issue.
A. Manhole cover is not correct size for manhole and is not stable or safe for pedestrian traffic. I fell through and injured myself walking across it.
