many potholes, some small and most larger and deeper all along street but seem to concentrate in the last three blocks closest to the hospital. ambulances are up and down the road day and night and these potholes are proving a life threatening hinderance to emergent medical care!

If someone is driving down this road fast enough to get in an accident and die, they are driving far too fast. I think in driver's ed they taught us to drive according to conditions - a road full of potholes doesn't allow for even driving the speed limit, much less fast enough to crash your car and die.
Perhaps you'd be better suited to notify public works, since they are the ones who patch potholes, and not rely on a website that has nothing to do with the city departments.
