This is an L.E.D. luminaire which is on the EASTBOUND SIDE OF E. HURON, opposite 809-811 E. HURON. Along with other units between Fletcher and N. State this unit recently had power restored. The L.E.D. segments are cycling on and off alternately, as if some sort of power supply failure.

11.29.2021 **URGENT** Street lighting on both sides of E. Huron between Fletcher and State not operating. Night visibility poor which is very risky for pedestrians crossing at the designated crossing adjacent to Rackham Bldg.

10.17.2021.SU Pole/overarm mounted H.P.S. luminaire to the south of 2895 Elmwood: Not illuminating.

Location is on east side of Bellwood across from 2906 Bellwood. H.P.S. luminaire is overarm mounted on newer pole with medium voltage conductors supported by top-mounted crossarm. The lamp housing access panel bears the numeral "10". At night the lamp emits a very dim blue "glow" but does not fully ignite. (See photo)

High pressure sodium, bracket/pole mounted, located appr. 60 feet from Oakwood St. on west side of Bellwood Av. adjacent to 2642... BEGAN TO FAIL ABOUT 11.18.2020, FAILED TO OPERATE AT DUSK ON 11.24.2020. UNIT IS NOW INOPERATIVE.

Driving southbound on Platt Rd. approaching intersection of Platt & Oakwood St., street name sign is blocked from view by vegetation. Sign can only be seen when in the intersection itself. THIS IS A SAFETY ISSUE and is a manifestly dangerous condition for drivers AND pedestrians, especially at night. THIS IS THE SECOND OF TWO SAFETY ISSUES CONCERNING THIS INTERSECTION. The FIRST one is called out in REQUEST No. 6587646.

SOUTH side of Lookout Cir., WEST side of Platt rd., Pole-mounted High Pressure Sodium Luminaire NOT OPERATING. THERE IS NO D.T.E. IDENTIFICATION PLAQUE ATTACHED TO THE POLE (D.T.E. Pole No. M17AA795). This is a TRAFFIC and a PEDESTRIAN safety issue!
There is practically NO VISIBILITY LEVEL for turning left on to Oakwood from Platt (southbound on Platt) after dusk!!! Vegetation issue will be reported separately.

THIS IS DTE PLAQUE NO. S10AE316. IT IS A HIGH PRESSURE SODIUM LUMINAIRE WHICH IS POLE-MOUNTED AND IS ON THE NORTH SIDE OF EDGEWOOD DR. ADJACENT SCHEFFLER PARK. HEADING EAST, IT IS THE FIRST UNIT EAST OF PLATT ROAD. THIS UNIT IS NOT OPERATING, CAUSE UNKNOWN.

THIS UNIT IS NOT OPERATING

(DTE Plaque No. S10AE337) This is a post-mounted, globe-type luminaire, probably metal halide lamp. It's set back from the sidewalk about 6 feet and is between N. University and 410 Observatory. It is just west of an AATA ("The Ride") bus stop. It is partially concealed by some shrubery. IMO this a safety issue for those waiting for buses.

THE FOLLOWING H.P.S. UNITS IN ADDRESS RANGE 717 THROUGH 920 E. HURON WERE FOUND TO NOT BE OPERATING AT ABOUT 9:45 pm MONDAY, 10.8.2018. They are: S25AG853 (717), S25AG854 (805), S25AG858 (920, Fletcher), and S25AG859 (902). Also refer to reported issue, ID 4996654, serv. req. 347866. There is much foot and vehicular traffic in this area throughout the evening hours, and with winter coming it can be very hazardous to walk/ride/drive here. The adjacent L.E.D. luminaire units on U of M property to the south do not provide sufficent illumination.

9.5.2018 PLEASE understand that this is not any sort of attempt to place or assign blame to any individual. Damage to recycling cart No. F0324448 has occurred two weeks in a row at this point. I've had to use a 3 inch rubber mallet to "bump" it back into shape both times, and I'm not sure how long I'll be able to do that without scrapping the cart. Is this due to the age of the unit or to some relief setting in the hydraulic system of the truck in question? My rubber mallet thanks you for your attention.

Southeast corner, traffic signal column, needs to have terminal enclosure cover reinstalled ASAP!!!!!

Luminaire assembly detached from overarm and lying on ground next to post. Wires are out the end of the overarm about 8 inches. (This might indicate work in progress to be completed after temperature has risen).

DTE No. S10AF556, H.P.S./H.I.D Luminaire not operating. Location, N.E. corner, intersection of Nixon and Bluett. THIS IS A CRITICAL LOCATION WHICH IS ADJACENT TO A MARKED, UNREGULATED, UNSIGNALED PEDESTRIAN CROSSING. Driving Southbound on Nixon I could not clearly see a pedestrian waiting to cross west-to-east, Time was about 6:40 P.M., 12.5.2017.

Eastbound Miller about 200 Ft. West of Newport: green rectangular sign NEWPORT RD. partially obscured by new utility pole installed to replace the one on which the sign is mounted. I have missed the left turn to go north on Newport for having not seen this sign.
Need sign to be mounted on the new pole so as to be unobstructed. This can be a difficult intersection as it is, particularly on sunny mornings.
