    I have submitted firmer requests for this property which have been closed without investigating. Officer Rast has conducted inspections for other Property Safety Code Law Violations with enforcement implemented. I have MS. On 03/02/2019 & 07/14/2019 I tripped over uneven, cracked, broken areas in the major cement walkway in the backyard of our home rental property, which is approximately 75% of the back yard encompassing the in ground pool. It is impossible to walk in the back yard without walking on these areas. I sustained serious injuries, including a permanent disfiguring disangulation of my coccyx & fracture of my S4 Sacral spine, disc herniations in my neck & lower back which required 3 separate spinal surgeries. Three Personal Injury Liability claims have been filed for myself & another individual injured from the damaged walkway. I’ve submitted several repair requests to our Landlord with contractor assessments & repair appraisals, all of which have been consistently denied by our Landlord. I am moving out this weekend. As a RN I have a legal, moral & ethical obligation to do no harm & report anything that presents as a health/safety hazard. I don’t want the next tenant who lives here to sustain the serious injuries that I have, therefore I am submitting one last request for the property to be inspected with Property Safety Code Law Enforcement to repair this property prior to any new occupants moving in.

    The roof is in poor condition. Various areas of tile were damaged during the hurricane and have never been repaired properly. Persistent leaks have resulted in internal damage to the main structural wall at the front of the house and in the lanai at the back of the house which has resulted in personal property damage. Water appears to be trapped within the main structural wall at the front of the house and has leaked below the wood floor which is now rotted and breaking apart with visual black mold. It is suspected that there may be black mold within the wall also. There is also external damage to this same wall where insects and rodents have gained access into the home. The cement walkway in the back yard around the inground pool is severely cracked and broken in various areas making it a major safety hazard. I have a permanent physical disability and have filed numerous formal maintenance repair requests with our Landlord Home Partners through Pathlight Property Maintenance which have been denied under the basis that the property is rented "As is". In March 2019 I sustained a serious physical injury when I tripped over a cracked broken area in the cement walkway in the back yard and fell. I had spinal surgery related to this injury that I sustained when I fell. I informed the landlord of this injury with another formal request that the walkway be repaired. The formal maintenance repair request was denied again. The persistent refusal to make these necessary repairs appears to be in violation of rental property safety codes. Home Partners also refuses to provide me with the homeowners insurance company information to enable me to file a claim for this injury. As a result I have over $50,000 in unpaid medical bills related to this injury. While recovering from my spinal surgery, I tripped and fell again on the property resulting in another physical injury which requires additional medical treatment and surgical intervention. I have also requested disability guard rail installation along the steps that enter into the inground pool because the cement that secures the ladder into the pool in cracked and damaged and the latter is not secured safely. A work order was also submitted for this and was denied. This seems to be a form of discrimination against a person with a disability. I truly appreciate any assistance you may be able to provide to help facilitate and enforce compliance with implementing these necessary repairs to our home.

    The condition of the pool is substandard. The surface of the pool is eroded with sever micro-organism infection due to cordon and cement seeping through various cracks and crevices in the pool surface. The condition of the pool presents a health and safety risk in its current condition. We are tenant and service maintenance requests have been med by us and the contracted pool cleaning vendor and the homeowner/landlord (HomePartners/Pathlight Property Management) have defused to make repairs.

    This is rental property. On 06/07/2017 a Maintenance Repair Request was submitted to the Landlord/Owner Homepartners aka PathlightMaintenance Property Management for repair of a roof leak. This is in the exact same area of leak that caused substantial property damage in 09/2016. A worker was dispatched & a tarp was placed on the roof. To date no repairs have been made despite several email & telephone requests for repair. As a result there is now internal property damage to the ceiling in the kitchen of the home & suspect additional internal water damage in the attic. There are two others areas of leakage in the bedroom & Lanai. Property owner/landlord refusing to make repairs.

    The wood deck in the backyard surrounds a pool and hot tub is a major walkway that leads out to the pinellas trail. The deck has several areas where the wood has become rotted with an unstable base, splintered wood with several areas of hole in the walkway & exposed rusted nails. The property owner is aware. Three different inspectors have been out to the property & submitted official repair requests with estimates. The property owner has persisted in refusal to make the repairs despite the severe safety hazard. Two days ago a person was injured on the property when her foot went through the rotted wood sustaining a head concussion among other injuries. She was treated at a local ER. Homeowner was notified of the injury but still refuses to repair the damage walkway.
