The sanitation (garbage) truck that serviced Venetian Isles (in particular Michigan Avenue NE) late in the afternoon on Friday, 12/1 sprayed a large amount of hydraulic fluid on the payment in front of most every residenc (including mine . . . at 2028 Michigan Avenue NE)e It appears most of the fluid was sprayed on the pavement when the truck was stationary while lifting/dumping garbage tote. The attached picture was taken in front of my residence. Please inspect and repair this vehicle. Note: This is not the first time one of your vehicles has defaced our beautiful neighborhood by spraying hydraulic fluid on the payment. Note also: I was unable to close/submit my complaint without picking an issue being reported. Unfortunately "other" was not available.

Contrary to close-out report, previously reported ~6" deep x 12" pothole in the center of the road at the intersection of Bayou Grande Blvd. NE and Grand Canal Blvd. NE has not been filled/patched and is still a hazard. Previous submission ID #15628615.

Special Pickup Request: Request curbside pickup of waste ~2' x 4' metal-framed glass casement window at 2028 Michigan Avenue NE, St. Petersburg FL 33703.

Stop sign on Bayou Grande Blvd. NE directly across from new home construction at 3-way intersection of Bayou Grande Blvd. NE, Shore Acres Blvd. NE, and Grand Canal Blvd NE has been struck (possibly by a construction vehicle) and the pole is bent at a ~45 degree angle and the stop sign is facing ~90 degrees to the left making it difficult for drivers on Bayou Grande Blvd to see . . . creating a dangerous safety issue.

Overall, the new 40th Avenue NE bridge is a huge improvement! However, both of the green under-bridge centerline lights need to be recessed/raised up so as to not impede the flow of tall boat traffic. As is, both under-bridge green centerline lights (one on either side of the bridge) hang down approximately a foot below the underside of the bridge effectively reducing clearance by approximately a foot. Please correct. Thank you.

Strongly recommend installation of pedestrian-operated bright amber crosswalk warning lights at the crosswalk on Bayou Grande Boulevard NE adjacent to Huntington Street NE in front of the Shore Acres Mini Park playground. This is a high-traffic low-visibility area on the blind side of a curve and adjacent raised bridge. I really don't understand why attention-getting flashing amber lights have not previously been installed here.

Approximate 2'X2' section of asphalt missing on Overlook Drive NE on north side of traffic hump in front of 4717 Overlook Drive NE. Previous patch attempt failed and asphalt continues to deteriorate/disappear.

-12" wide x 6"-8" deep hole in asphalt roadway directly in front of 1711 Shore Acres Boulevard NE (just north of Huntington Street NE). Note: Previously reported (incorrectly) as 1700 Shore Acres Boulevard NE.

~12"-18" wide x ~6"-8" deep depression just off center of roadway directly in front of 4735 Shore Acres Boulevard NE. Cars swerving to avoid depression are creating a dangerous situation.

~6" triangular section of asphalt pavement missing from corner of previous patch in center of Grand Canal Boulevard NE adjacent to canal between Hawaii Avenue NE and Carolina Avenue NE in Venetian Isles.

Please check the fleet of garbage and recycling trucks for a blown seal (either oil or hydraulic fluid). I ask as either a Garbage or recycling truck is once again spewing fluid (most likely hydraulic fluid) in the streets throughout Venetian Isles . . . and I assume elsehere.

Replace downed/damaged decorative combination stop sign/"Historic Uptown Neighborhood" pole directly in front of St. Anthony's Hospital main entrance at intersection of 7th Avenue North and 12th Street.

Approximately 18" diameter x 8" deep depression in Shore Acres Boulevard NE near centerline causing cars to dangerously swerve.

Multiple streetlights out/inoperative on south side of 5th Avenue North between 10th Street North and 14th Street North (adjacent to St. Anthony's Hospital). Note: These streetlights are not included in Duke Energy's automated reporting system; therefore, I assume they belong to the city.

Relatively new pothole in southbound lane of Overlook Drive NE just prior to intersection with Connecticut Avenue NE.

Please reconsider previous decision to not install illuminated crosswalk lights at the 5th Avenue North crosswalk from the southwest corner of the St. Anthony's Hospital Jackson Street Garage to/from the parking lot between 5th Avenue North and I-375. Although close to another illuminated crosswalk, this crosswalk is very dark and is used much more frequently than the other crosswalk. Please re-evaluate previous decision.
