Brookhaven + Park

Rubber flooring needs to be replaced trip hazard for kids and adults.

Park grounds - the rubber is terrible, it's coming out and in some cases completely gone from under the swings making it dangerous.
The park also needs to be covered if possible, during the day it's dangerously on all of the equipment. We as a community have made numerous of similar requests with no action. Please advice on next steps and how to make this a more formal request for action.

The drain next to the water fountain at south buena vista dog park is clogged and overflowing

Many of the bolts have come loose or completely fallen out, that are holding the playground fixtures together, that the kids climb on.
At least 4+ have completely fallen out and are now missing
At least 12+ are loose and need to be tightened

Sewage is bubbling up and leaking from a manhole at 2205 S Melrose Dr.

There is more than enough room for at least one bench on the south side of the playground between the swings and slides. One bench is 100% more than no bench :) Please and thank you!

The outhouse at the South Buena Vista Off Leash Dog Park needs to be ADA accessible. It currently is very difficult to use for individuals with mobility issues.

Green light is burned out on southbound Melrose/Sycamore intersection.

Large chunks of rubber surfacing at South Buena Vista dog park playground are missing. So much is missing under the two big swings that it's really hard for kids to get on and off.

Irrigation pipes broken and water slowly leaking from box

Faded "NO PARKING ANY TIME" sign at the beginning of westbound Faraday Ave on the right side just west of S. Melrose Dr.

Deep pothole in the #2 lane of southbound S. Melrose Drive roughly halfway between Sycamore Avenue & Oak Ridge Way.
