Holladay and Millcreek

I live in Holladay and frequently enter the onramp from Big Cottonwood Rd (6200 S in Holladay) to NB I-215. The signage indicates that the traffic from Wasatch Blvd entering the same onramp should yield to the onramp from 6200 S in Holladay. People entering from Wasatch Blvd are often going very fast (especially during ski season) and frequently don't yield to the other vehicles entering onramp in spite of yield signs. I have seen >30 near-accidents in the past few years (including me and my wife). I believe two flashing yield sign would be appropriate to let the Wasatch lane see that they are to yield to the lower onramp with a blind spot.

I received the following reply from UDOT, but no action has been taken:

"In Numetric (crash history program) this northbound onramp to I-215 from Wasatch Blvd and Big Cottonwood Road shows a crash history of multiple front to rear and sideswipe crashes. I have put in Work Order #2017-18 instructing the Sign Crew to replace the existing outside shoulder "Yield" sign with a flashing "Yield" sign and to also add a "Yield Ahead" sign on each side of the road roughly 300 feet prior to the flashing "Yield" and existing regular "Yield signs. These new signs should cause the traffic that is required to yield to actually yield."

Please make the corrective actions as soon as possible

This poor deer has been here for over a week, right before the 3300 S overpass, already called once. Please pick up the poor creature.

Multiple potholes combined with pavement deterioration has created a very rough “off road” like travel way through the intersection. Issues throughout the intersection as a whole, but worse seems to be concentrated in lane #2 of both SB and EB (southwest section of the intersection).

Recent construction was done by children. The Service access (manhole) is 4 inches below the NEW road surface.

Heading northbound on 1300 East, when you turn left onto Van Winkle, the road needs to be repaired. Feels like your tires are going to shred when going over it.

There is a growing pothole in front of my driveway that is beginning to cause the cement curbing to drop and crack. This pothole is becoming a problem for cars passing over it. Can it be fixed.

We were driving and started to slow down because the lanes were merging and because we were focused on merging, we did not see the huge massive crater of a pothole. We dropped into the hole and the car shook. Immediately, we noticed that the front left tire was completely flat. As we were changing it, we noticed a huge hole in our tire. Not cool, needs to be fixed.

This is a major intersection. Traveling north on 1300 East and making the left onto Van Winkle, you have to steer quite far across the intersection to avoid the damaged road. It's been this way for many months.
