Previously patched potholes need repairs again please

There are many potholes between 205 and 206 Pratt Ave in the back of the road before the parking spots.

near 34 Newhall Street driveway / on Crosby Street - potholes in road and just before turning into condo parking lot.

There is a stub of a street sign post sticking about 2 inches out of the sidewalk. My wife tripped over something like this and smashed her face. She was told that the city relies on people to report these things. Here’s your report. Thank you

I ordered take out in the first week of January 2021 and while waiting for my order I noticed the chef was not wearing any gloves and had long sleeves almost going in the food. The sandwiches were good but because the Chef was not wearing gloves I will not go back. Please look into this.

Stop sign fallen down into street.

This is a very dangerous intersection. Cars are constantly running the traffic lights and even beeping at pedestrians while they have the right away. I am legally blind person and feel very unsafe.

The crosswalk signal’s do not work properly on all four corners of the intersection.

Three pot holes on street

Bumps, frost heaves, poor work on multiple area of the Lowell connector. Across all three lanes at the top of the on ramp going out of Lowell. (thorndike street entrance)
Also on the other end mid lane mostly when the lanes merge into three lanes - going towards lowell (near 495 entrance from the connector)

On Chelmsford Street, the right lane is a turning lane only, but the marking are faded and not visible. Many drivers use that lane to go straight across Plain Street not knowing that they are in the wrong lane. It's extremely dangerous and the lanes need clearer markings.
