Several large potholes on exit ramp and at end of exit.. Some of the holes were filled at beginning of ramp but from middle to end lots of large holes...

If the pot holes are on Seaford Oyster Bay Expressway exits you need to call the New York State Dept. of Transportation: 1-800 POTHOLE.
If the pot holes are on Old country road you need to call Nassau County DPW: 516 571-6900
