I want to file a damage claim with DWSD regarding 3ft of water flooding my basement. I had over $40,000 worth of damages. I lost my Furnice, hot water tank, furniture, bedroom furniture, clothes, shoes, and my electrical outlets were damaged. My wall and floors were damaged as well Please see attached claims form. I was only allowed to upload 1 picture. I have several contractors estimates and pictures of my damaged/loss items

R. I contacted customer service several times for assistance with up loading my damage claims form and was told by several customer service reps, Ms Barber and Ms Bradshaw that they weren’t given any instructions how to help peo complete/upload forms
