Respectfully disagree with your assessment that this meets MUTCD. Figure 9C-4 seems to be the closest I could find to an example. See photo.
More importantly, the Complete Streets Ordinance adopted January 2015, says designs for projects like this are supposed to follow NACTO, not just MUTCD.
