Found this issue on February 26, 2018.
Eastbound side of Grand Street by Marin Blvd intersection. The metal collar is no longer attached and the cover is missing. This is the ~ 12" water / gas access.

Thank you for your request. Potholes referred to the Division are normally processed within 48 hours.
Sinkholes will be referred to the appropriate utility agency for inspection and repair.
Please note that potholes cannot be repaired on days with heavy rain because the moisture will prevent asphalt from bonding with the existing pavement.
