11/14/14-Friday: A walk through was conducted on line and the form is accessible. It just takes several clicks to get to the page. Click on Department - scroll down to Courts - then scroll down to Probate Court - click on Marriage License application. Under forms click on marriage license. The PDF version of the form will appear.
