J. Barker investigated 9/10/18. Inspected inlet structure and found no defects. Will need to schedule a video pipe inspection to check for pipe defects. Made area safe by placing traffic cones around affected area to warn the public.

Area made safe until repairs can be made. 9/10/18 J. Barker

Personnel added flagstone to hole to make safe. Will be video inspected.

Sinkhole near storm drain has partially re-appeared.
