What a nightmare! More incompetence.

This has been registered as three separate service requests - this needs immediate attention. The requests are: 550077, 550349 and 550240.

This was patched again but is now failing again - please send crew out to repair. Same goes for the patch up a little ways (on MacArthur) in between Park and 13th Ave.

This is another incredible bad patch job that obviously failed miserably!! I understand PG&E open that up but shouldn't PG&E be accountable for a patch job that should've done. Why do a continuous patch job it's going to fail especially in the very severe rains. To just fix it right the first time that the tenth!! I almost got hit yesterday try to avoid the majority of the hole.

Dear DPW: "Temporary patch at valve cover repair has failed, leaving a 4x4 hole in the pavement and gravel strewn across the bike lane. City needs to inspect PGE work here." --I'd vote to fix, but that option's been closed! :-(

This issue was addressed today by OPW crew Photo attached was taken about 4pm today 12/18/2014. This issue was resolved by one of the multiple trouble reports that were already open on this issue.
