Confusingly, Wade Avenue is maintained by the NC DOT, not by the City of Raleigh. You can use this form to report potholes in Wade Avenue: https://www.ncdot.gov/contact/Pages/form.aspx

I've found them to be very responsive, but note that you do need to enter a separate request for each pothole, which means that you need to identify the street address of the closest house that is actually on Wade. If you tell them the closest is one of the houses that face Hymettus Court, they will kick it back and tell you that they don't maintain that street.
